  • Complete physical exam
  • Nutrition consultation with a Registered Dietitian

  • Lifestyle assessment and personal goal setting
  • End-of-day consultation with physician to review assessment results and discuss findings, action plans and recommendations

SHIFT is located on the southwest corner of N. Orleans St. and Chicago Ave. Metered and lot parking is available within walking distance. To pay for metered street parking, use the Park Chicago app. Use Spot Hero for surface lot and garage parking in the vicinity.
