Member Services Associate

SHIFT is a new model of primary care.

SHIFT is a group of passionate and committed experts who believe in a more intelligent, more effective approach to delivering personalized healthcare and helping people build health. Our medical care is the foundation of what we do; we care for members during times of sickness and health.

Our nutrition, fitness and recovery experts work in tandem with our primary care medical team to help members develop the necessary skills to shift thoughts and actions in order to confront and conquer challenges in our ever-changing lives. We use medicine to address what cannot yet be achieved through lifestyle modification. We use diagnostic tools to measure quantitative progress. We help members focus their efforts on goal setting and habit formation and use our proprietary coaching methodology to guide members toward specific health milestones. We prepare members to make the positive, lasting changes they desire. We are their ally and advocate. We listen to our members and care about their aspirations. We believe the achievement of optimal health and performance is an ongoing, evolving pursuit. We have dedicated our lives to improving the lives of others. ROLE OVERVIEW:

Working closely with our Sales and Operations Manager, you’ll be responsible for providing ancillary support to our Sales and Member Services team. Your role will require involvement in various workflows related to Member Services, Facilities Management, and Sales. Your main objectives of this role are to provide exceptional customer service to our members and support the Sales team in the end-to-end sales process. You’ll ensure problems are solved efficiently and professionally and member needs are addressed quickly. The position is based in the River North area of Chicago, IL and requires a 6:15a start, Monday-Friday.

CORE FUNCTIONS:

  • Greet and assist SHIFT Members and visitors daily
  • Maintain a clean, modern, functioning, efficient, and inviting facility for members, guests, employees, and vendors
  • Provide support with implementation of operational improvements and sales activities
  • Manage various functions related to Membership transactions and service delivery
  • Share materials, answer questions and conduct tours for prospects, as needed
  • Deliver outbound communication to members on a scheduled and ad hoc basis that is free of grammatical and formatting errors
  • Provide tactical support for Marketing and Communication efforts
  • Support the SHIFT Fueling Café
  • Deliver inbound mail and packages
  • Run local errands, as needed
  • Address routine and ad hoc facility upkeep issues
  • Manage a broad range of core administrative duties in support of Operations
  • Be challenged to acquire new competencies and accept new challenges

EDUCATION/EXPERIENCE:

  • A bachelor’s degree
  • 2-3 years of experience in a dynamic, demanding environment

Registered Dietitian

ROLE OVERVIEW:

At SHIFT, we are looking for a Registered Dietitian, who is an exceptional clinical provider, detail-oriented, comfortable being part of a small, fast-paced team, and confident in their ability to handle a wide range of responsibilities on a day-to-day basis. Our mandate is to help Members build, expand, and maintain complete well-being—our integrated, relational healthcare model drives stronger physical performance, increased energy, improved mental acuity, and positive health outcomes.

The Registered Dietitian plays a vital role in healthcare services and provides our Members Medical Nutrition Therapy, nutrition counseling, and habit coaching. The Registered Dietitian is responsible for working collaboratively with subject matter experts and the medical team to provide holistic, integrated care plans for our Members. At SHIFT, the Registered Dietitian’s role also involves developing educational materials, running the SHIFT Fueling Café, and managing Member data.

CORE FUNCTIONS

  • Consistently exhibits behavior and communication skills that reflect SHIFT’s commitment to superior customer service, including quality, care, and concern for every Member.
  • Participates actively in SHIFT Member meetings. Documents pertinent Member information, all communication, and special circumstances of Members. 
  • Follows all state and Federal statutes, regulations, and laws related to privacy, Protected/Personal Health Information (PHI) and HIPPA
  • Provides evidence-based nutrition counseling and adheres to the Code of Ethics, as stated by the Academy of Nutrition and Dietetics
  • Counsels according to the SHIFT Fuel Philosophy
  • Provides nutrition counseling and Medical Nutrition Therapy to Members (in-person, by phone, and via email)
  • Analyzes and shares results from nutrition-related testing: DEXA Scans/ /Labs
  • Uses technology and templates to prepare and present data and test results
  • Collaborates with other subject matter experts/physicians, to ensure that safe and appropriate action and care plans are delivered in a clear, consistent, and coordinated manner
  • Works collaboratively with all Nutrition and other SHIFT staff to ensure high-quality, professional service delivery
  • Develops nutrition education materials
  • Supports SHIFT Fueling Café (source, order, label, stock, and track items) operations
  • Performs administrative functions in support of day-to-day operations
  • Supports special events

EDUCATION

  • A bachelor’s degree (and/or master’s degree) in nutrition and/or dietetics or other relevant major with completion of didactic program in Dietetics
  • Successful completion of Dietetic Internship approved by the Commission on Accreditation of Dietetic Education
  • Credentialed as a Registered Dietitian (RD) by the Commission on Dietetic Registration (CDR)
  • Licensed Dietitian Nutritionist (LDN) by the Illinois Department of Financial and Professional Regulation in good standing
  • 3 years of clinical and counseling experience preferred

KNOWLEDGE/SKILLS/ABILITIES

  • A proven track-record caring for patients according to the Academy of Nutrition and Dietetics Code of Ethics
  • Experience and comfort with learning new online systems/software
  • Advanced knowledge of MS Office Suite and Adobe Acrobat.

Primary Care Physician

WHAT YOU'LL DO:

  • Deliver quality care for all patients, providing healthcare services that include but are not limited to comprehensive assessments, diagnosis, patient education, treatment, and referrals, as necessary. 
  • Provide comprehensive and ongoing care to a smaller patient panel, including in depth initial assessments, personalized health planning and high touch ongoing care. This includes a coaching-like approach to patient engagement. 
  • Assist Medical Director in refining medical policies, quality improvement processes, and evaluating current practices, policies and procedures.
  • Adhere to the highest standards of medical practice, ethics, and professionalism at all times. 
  • Contribute to open and honest ongoing communication with members and the team. 
  • Attend required meetings and participate in committees, as requested.
  • Handle confidential matters concerning clinic policies and practices in accordance with HIPAA regulations.
  • Maintain strict confidentiality.

YOUR QUALIFICATIONS:

  • Board certified physician with a valid Illinois practice license 
  • Professional clinical experience in specific area of practice (i.e., Internal Medicine, Family Medicine, Primary Care, Preventive Health or Executive Health)

YOUR WORK ENVIRONMENT:

  • Non-traditional concierge medicine environment, must be responsive to patient panel via texts/SMS/emails in a timely manner, staying well-informed of member status and needs. 
  • Available for regular on-site clinical hours and on-call responsibilities.
  • Rapidly growing practice, requires high energy and a commitment to learning, development and growth.
