Clinical & Administrative Care Coordinator

SHIFT is a new model of primary care.

Our nutrition, fitness and recovery experts work in tandem with our primary care medical team to help members develop the necessary skills to shift thoughts and actions in order to confront and conquer challenges in our ever-changing lives. We use medicine to address what cannot yet be achieved through lifestyle modification. We use diagnostic tools to measure quantitative progress. We help members focus their efforts on goal setting and habit formation and use our proprietary coaching methodology to guide members toward specific health milestones. We prepare members to make the positive, lasting changes they desire. We are their ally and advocate. We listen to our members and care about their aspirations. POSITION DESCRIPTION:

The Clinical & Administrative Care Coordinator will support the medical team with various administrative tasks in an excellence-focused outpatient primary care setting. The Care Coordinator will be responsible for facilitating a continuum of patients’ care utilizing advanced knowledge, experience, and skills to ensure the appropriate utilization of resources.

We seek a seasoned, personable, and professional team member. Compelling candidates thrive in a fast-paced, collaborative environment, and have superb customer service and organizational skills.

PRIMARY RESPONSIBILITIES:

  • Consistently exhibits behavior and communication skills that demonstrate SHIFT’s commitment to superior customer service, including quality, care, and concern with each and every member.
  • Fulfills organizational responsibilities efficiently as assigned which may include but not be limited to:
    • Administrative Responsibilities:
      - Inventory management
      - Facilitate efficient receipt and distribution of communication via phone,
         fax, email, internal digital databases, and physical documents.
      - Facilitate physician medical prior authorization process
    • Care Coordination:
      - Scheduling outreaches
      - Administrative facilitation of medical referrals
      - Member access coordination
      - Coordination of quarterly off-site physician offerings
      - Facilitation of daily member service delivery, from arrival and check-in
         through post-visit follow-up.
  • Working as a team member; recognizes problems with patients, staff, or operations and works quickly to resolve the problem. Identifies ways to improve practice operations.
  • Uses, protects, and discloses SHIFT patients’ protected health information (PHI) in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
  • Maintains effective communication with physicians, SHIFT care team members (medical and extended), related service vendors, external care providers, hospitals, patients and, when appropriate, patients’ families.
  • Implements, monitors, and recommends improvements to care management programs and practices within policies and procedures set by the Lead Physician.
  • Provides accurate information to patients and families regarding membership benefits, resources, referrals, medications, treatments, and other related issues.
  • Performs other duties as required or directed by providers and management.

QUALIFICATIONS:

  • Bachelor’s degree from a four-year college
  • 3+ years of Administrative, Executive Assistant, or Care Coordination experience preferred.
  • EPIC electronic medical record experience preferred.
  • Strong customer service skills
  • Ability to perform all work in compliance with all laws and regulations.Excellent written and verbal communication skills.
  • Uses professionalism and diplomacy in interpersonal interactions with patients, guests, and staff.
  • Able to learn, retain, and accurately use information regarding patient care policies and procedures.
  • Comfortable working within all Microsoft Suite products and Adobe Acrobat
  • Works well in a team, bringing and encouraging positive energy
  • Able to adapt to change
  • Diligent, resilient, positive, and articulate
  • Able to oversee multiple projects and balance competing priorities

Executive Assistant

POSITION DESCRIPTION

The Executive Assistant will support the Chief Operating Officer (COO) directly as the primary driver of business momentum to progress toward critical goals. The EA will be responsible for the management and operational support of the office, assist with special projects, and provide key leverage to the COO, ensuring the highest and best use of each team member.

We seek a seasoned, personable, and professional team member, who is a critical thinker and efficient doer. Compelling candidates thrive in a fast-paced, collaborative environment, and have superb teamwork, interpersonal, and organizational skills. The COO will depend on this person to build systems to effect organizational change and to reach strategic objectives.

PRIMARY RESPONSIBILITIES:

A) Executive Management

  • Drive COO’s connectivity and productivity by facilitating communication with multiple stakeholders
  • Support the executive staff/team through development, escalation, triangulation of information and resources
  • Serve as an operational backbone for the executive team, clearing the path and anticipating challenges
  • Manage the executive’s calendar, including making appointments and prioritizing the most sensitive matters to ensure the highest and best use of COO time
  • Provide added leverage by identifying and completing tasks proactively
  • Manage and protect confidential information and keep confidentiality

B) Administrative and Clerical

  • Maintain accurate, easily accessed records, notes, task lists, and calendars for COO
  • Handle schedules and coordinate meetings, as needed
  • Prepare proper materials prior to meetings, manage meeting notes, and follow up on key takeaways and to-do items
  • Write and edit e-mails, draft memos, and prepare communications on the COO’s behalf
  • Organize and oversee planning for events, as needed
  • Work with third-party service providers and vendors, as needed
  • Run local errands and provide support on personal matters, as needed

C) Project, Task, and Process Management

  • Collaborate cross functionally to plan, organize, and execute projects from ideation to delivery and measurement
  • Prioritize tasks based on current activities, goals, and deadlines
  • Drive and manage annual plans to reach objectives and hit metrics
  • Manage processes across a range of initiatives and ensure quality and prompt completion
  • Transfer information between teams, create meeting summaries and to do’s, ensure meeting preparedness
  • Communicate agendas, goals, and processes to team
  • Oversee and be the ultimate checkpoint for coordination and logistics for the team, as needed

D) Analysis

  • Prepare and provide accurate reports on key metrics based on raw financial and operational data
  • Create/Maintain performance dashboards in multiple business areas
  • Independently develop written and visual analyses of information
  • Gather and synthesize information for review
  • Provide clear visualizations of information and offer insights

YOUR QUALIFICATIONS:

  • Excellent written and verbal communication skills.
  • Strong organizational and time management skills
  • Strong experience in using Microsoft Excel (calculations, graphs, tracking, financials)
  • Comfortable working within all Microsoft Suite products and Adobe Acrobat
  • Working knowledge of Microsoft Power Platform, especially Power BI preferred
  • Works well in a team, bringing and encouraging positive energy
  • Able to adapt to change
  • Diligent, resilient, positive, articulate, and professional
  • Able to produce reports, memos, and other inputs to inform critical decision-making
  • Able to oversee multiple projects and balance competing priorities

WHAT YOU'LL DO:

  • Deliver quality care for all patients, providing healthcare services that include but are not limited to comprehensive assessments, diagnosis, patient education, treatment, and referrals, as necessary. 
  • Provide comprehensive and ongoing care to a smaller patient panel, including in depth initial assessments, personalized health planning and high touch ongoing care. This includes a coaching-like approach to patient engagement. 
  • Assist Medical Director in refining medical policies, quality improvement processes, and evaluating current practices, policies and procedures.
  • Adhere to the highest standards of medical practice, ethics, and professionalism at all times. 
  • Contribute to open and honest ongoing communication with members and the team. 
  • Attend required meetings and participate in committees, as requested.
  • Handle confidential matters concerning clinic policies and practices in accordance with HIPAA regulations.
  • Maintain strict confidentiality.

YOUR QUALIFICATIONS:

  • Board certified physician with a valid Illinois practice license 
  • Professional clinical experience in specific area of practice (i.e., Internal Medicine, Family Medicine, Primary Care, Preventive Health or Executive Health)

YOUR WORK ENVIRONMENT:

  • Non-traditional concierge medicine environment, must be responsive to patient panel via texts/SMS/emails in a timely manner, staying well-informed of member status and needs. 
  • Available for regular on-site clinical hours and on-call responsibilities.
  • Rapidly growing practice, requires high energy and a commitment to learning, development and growth.
